Examinations booked online are performed on:

Monday: 10am-6pm
Wednesday, Thursday and Friday:  10am - 5pm
Saturday:  9am - 3pm

1.   Print and complete the PATIENT HISTORY FORM. 
2.  Below, fill in the required fields.  In the "comments" section, give us the best phone number   to contact you back to confirm your appointment.  Your Phone number must be given or
else we cannot process your request.
3.  Give us your most desired appointment day and time.       Also give an alternative day and time. 

4.  Online Appointment Scheduling is limited to the following exam times: Monday: 10am-6pm; Wednesday,
Thursday and Friday: 10:00am - 5:00pm;  Saturday: 9:00am - 3:00pm  ALL on the 1/2 hour (ex 10:00am, 10:30am, 11:00am, 11:30am, etc.)

5.  Do not choose an appointment day within 24 hours of your request.  Schedule your appointment at least two days from the date of submission.

6.  Submit Form.  We will contact you back within 24 hours to confirm your appointment day and time.

7. **Bring to the office your current eyeglasses (even if they look funny or are scratched!) and any contact lens package labels or written contact lens prescriptions.
